The payment is by credit card or exam voucher. Exam voucher are provided if you sign up for course at those teaching course center, and depending on your course package, the exam voucher are included.

Woo ..... haven't you decided the path on exam ???
Decide exam first ----- study it ----- call 考试中心报名 ---- take exam (and know result/score after exam) --- wait for certificate

Score will be come out after exam ...... certificate will be issued after 4 to 6 weeks after exam !!

No ..... training center will give exam voucher (as gift) when you take course ..... try traininng in Official Training Center (But experience).
In your case (MCSD) ---- course should be taken ! (Self-study on this part exam is tough !!)
And also ..... practise and experience are keys on passing exam !!!

Whenever you have question on exam, come and ask !!!
By the way, you can try exam 70-210 (Win2000 Prof) or 70-270 (WinXP) .... and you are certified as MCP !!!
Once you have test, you know the standard on IT exam .... and then you can decide whether you continue MCAD !! You need to decide which programming language for MCAD, either C# or VB.
There are 2 core exams and 1 elective. You need to plan which which exam you are taking. Coz' some exam are required in other MS cert, when you make out the best combination, you can save yourself a number of exam.
Example MCAD needs 4 papers and MCSD needs 5 papers, if you plan carefully, you won't need to take 9 papers in total, as some papers are common between them.
